We look forward to seeing you soon. Masks are NOT required at this time for patients, staff will wear masks when actively involved in patient treatment.

Please read through the questions below and only call the office prior to coming to your appt if you answer yes to any of them. 

1.       Have you tested positive for COVID-19 in the last 10 days?  (If yes, when?)

2.       Have you knowingly been in contact with or exposed to any confirmed COVID-19 positive patients within the last 10 days?

3.       Do you currently have any symptoms of COVID-19?

4.       Do you have any other Cold or FLU-like symptoms?
